Welcome to the new Health Center website! I hope you find the pages in my site useful in providing nursing information. Some of the pages will be updated on a regular basis, and some will be changed only rarely.
It is my goal to make this a communication tool for your use. I will do my best to keep it current.
Health screenings will be held October 6, 2009. Rechecks will be done the following next three weeks for any questionable areas. You will be notified if you need to take your child for further evaluation of their hearing or vision.
PARENTS OF 6TH and 7TH GRADERS 11 years or older: Remember to turn in your medical records for the newly required Tdap & Meningococcal immunizations.
PARENTS OF 7TH and 8TH GRADERS: Your student is required to have a Varicella (chicken pox) immunization if they have not had the disease of chicken pox.

All immunizations MUST be current to begin attending school and kept compliant as additional immunizations may be required.
There has been a change in the SUSD guidelines regarding the over-the counter medication that is given out in our School Health Centers. You now need to complete a MEDICATION ADMINISTRATION RECORD, HMES 503, one form for each medication. Also, you must provide the specific, age-appropriate medication (as listed on the medication) and it must be in an original, sealed container before any over-the-counter medication will be given to your child. More information is listed under the MEDICATION POLICY, Important Information column below. Let me know if I may be of assistance for you bringing medication for your student.
